At 11:45 a.m., on January 24, 2019, two employees were inspecting the feed throat of the barrel in an injection molding machine that was having a material flow issue. The screw was retracted and overheated degraded nylon was forced out through the screw feed throat, burning one of the employees on the hands and face.
